My experience seems to be that it is push, though I will admit I havent fully tested it out.
I have had my iPhone for a few days now, and have setup mobileme and done a sync. I have set my iphone email on push. The Fetch setting is separate and I have set this to hourly.

I sent myself an email from my office to my mobileme address and in less than 10 seconds my iphone had received the message. I have done this several times in a row. This would seem to me that it is really pushing the email.

The Sync settings on the iMac would seem to be quite different and separate from the way the iPhone operates. I'm not sure I can think of a specific reason to make them different, but maybe because of the way email functions on most computers (pull oriented) is the behind it.
